How to Calculate Employee, Spouse, and Child Semi-Monthly Voluntary Life Premium
Locate the intersection of the age column to the total amount of life insurance (life amount column) requested to find
your semi-monthly premium. To calculate the Guarantee Issue amount of $300,000, you will add the semi-monthly
premium number in the $100,000 column to the semi-monthly premium number in the $200,000 column (or multiply
the $100,000 column by 3).
Employee Semi-Monthly Premium
Guarantee Issue = $300,000
Spouse Semi-Monthly Premiums
Guarantee Issue = $30,000
Child Semi-Monthly Premiums
Guarantee Issue = $10,000
