Life insurance and disability
 Life and AD&D Insurance
Excelligence Learning Corporation provides basic life and AD&D insurance for employees and offers voluntary insurance options for employees and their dependents.
Basic Life and AD&D Insurance
Life insurance is an important part of your financial wellbeing, especially if others depend on you for support. Excelligence Learning Corporation provides basic life and accidental death and dismemberment insurance in the amount of $25,000 at no cost to you. Coverage is automatic; you do not need to enroll.
Voluntary Life and AD&D Insurance
You may choose to purchase additional life and AD&D coverage for yourself and your dependents at affordable group rates. Rates are based on age and the coverage level chosen.
Voluntary Life and AD&D Insurance for you
Employee
● Increments of $10,000 up to 5x your base annual salary
● Up to a $500,000 maximum
● Guaranteed issue up to $150,000
Spouse
● Increments of $5,000 (not to exceed 50% of your voluntary life and AD&D coverage)
● Up to a $50,000 maximum
● Guaranteed issue up
to $50,000
Voluntary Disability Insurance
Voluntary disability insurance provides income replacement should you become disabled and unable to work due to
a non-work-related illness or injury.
Voluntary Short-Term Disability
Voluntary Long-Term Disability
Eligibility for the new Paid Family and Medical Leave Act (PFMLA) benefit begins January 2022 for employees residing in Connecticut. Please contact your local HR Representative for more information.
  Coverage
Benefit
● 60% of your weekly salary, to a maximum of $1,390 per week for the first 12 weeks of a disability after the one-week waiting period.
● STD benefits will be offset by other sources of income including state-mandated disability plans.
 ● 60% of your base salary, to a maximum of $6,000 per month if you are disabled and are unable to work for more than 90 days.
● Benefits are offset with other sources of income, such as Social Security and Workers’ Compensation.
       Family Medical Leave Act (FMLA)
If you have been with the company for 12 months, you may be eligible for up to 12 work weeks of unpaid leave per year under the Family and Medical Leave Act (FMLA). FMLA can be used for an illness of your own, care needed for a family member, care for a newborn and certain other medical needs.
 Voluntary Life and AD&D Insurance for your dependents
 Child(ren)
● Birth to 6 months: $500
● 6 months to 26 years: Units of $2,500 up to $10,00
● Cannot exceed 50% of employee life coverage
● Must be added within 30 days of birth
