ASSOCIATION WIDE FORMS
All forms should be filled out completely and thoroughly. All association documents are listed here and examples will be provided in camp trainings.
ACCIDENT & INCIDENT REPORTS
This form is used to communicate accidents or incidents to parents. Every single section on the form should be filled out completely. This document should include as much detail as possible and should not be handed to a parent without being reviewed by camp leadership. If the accident or incident involves more than one camper, please use the name of the camper the report is being written for. Finally, please use full names of campers and camp staff members on the form, no nick names etc. These forms should be filed in the designated place that your camp director has decided.
LIABILITY INSURANCE FORMS
Liability Insurance forms should be filled out with severe Accident and Incident Reports (head trauma, excessive bleeding, calling 911, etc). Every section should be filled out to the best of your knowledge, and your camp leadership will help fill out membership number, address, phone number, etc. There is a front and back to this form so please be sure both sides are completed. You can use all camper names involved on this report and be as detailed as possible. This is an internal form only and should not be given to parents to review. Give all Liability Insurance forms to your camp leadership team and they will send to Risk Management.
PARENT LETTERS
This should be used to communicate any severe incidents with parents/guardians. All parent letters should be reviewed by camp leadership before giving to parents/guardians to sign at pick up. It will be only camp leadership that can decide suspension or termination and they will discuss with the camper’s family.
REVIEWS & FEEDBACK
Our goal is to provide the highest quality camp experience to our families over the summer. To do this, we rely on constant feedback to our camp staff members. All counselors, activity instructors, etc. will receive one review Week 7 of camp. This review will include items like dependability, member experience, dress code, and initiative. All reviews will be scored on a scale of 1-5 with 5 being perfection in the area being graded.
We will also implement bi-weekly quick checks to measure performance as well as check in on the camp team. These will be opportunities to connect with your supervisor and discuss strengths and opportunities for both the staff member and the program as a whole.
