            PROGRAM REGISTRATION INFORMATION

         regarding communicable disease, and (iii) not participate in any event if participation would otherwise be in violation of any guidance issued by federal, state, and local
         health authorities. In the absence of signature, payment of fees and participation in the program shall constitute acceptance of the conditions set forth in this release.
