All Camps
Emergency Contact & Medical Information
Registration must include emergency contact and medical information to be considered complete.
Authorization for administration of medication is also required, if applicable.
Min/Max Number of Participants & Waitlist
Each camp has a minimum and maximum number of participants and these requirements vary from activity to
activity. A minimum number of participants is required in order for an offering to be conducted; the activity
is subject to cancellation if it does not reach the required number of participants. The maximum number
of students is determined based on the type of camp and the appropriate student to teacher ratio, so that
all participants may gain the full experience of the camp; a waitlist is available for activities that reach the
maximum number of participants.
Payment
Payment in full is required at time of registration; a $25 fee will be assessed for payment that cannot be
processed because of insufficient funds or a declined credit card.
Photographs
Providence Academy, its official representatives and assigns, have the irrevocable and unrestricted right to use
and publish photographs of all the Providence Academy Summer Activities participants for editorial, trade,
advertising, and any other purpose and in any manner and medium. Providence Academy and its official
representatives and assigns are released from all claims and liability relating to said photographs.
Supervision
All students in the building must be under adult supervision. If there is a span of one hour or less between two
consecutive activities that your child (age 5 - entering grade 6) is registered for, he/she may participate in PALS
Lunch Layover for a fee of $10/hour. Register at providenceacademy.org/summer
Weather
If a session must be cancelled due to inclement weather, or any other unforeseen circumstance, the instructor
will contact participants regarding the cancellation.
Withdrawal & Refund Policy
If your plans change and you wish to withdraw registration before May 8, the tuition, less a $50 administrative
fee, is refundable. To ensure staffing and program commitments to all families, no refunds will be granted
after that date, including withdrawals due to vacation schedules, change of plans, or conflicting activities.
Phone withdrawals are not permitted. Withdrawals must be submitted in writing to Dawn Schommer,
Manager of Auxiliary & Development Programs, at dawn.schommer@providenceacademy.org
