Short Term Disability – Lincoln Financial Group

Open Sky Wilderness offers you Short Term Disability insurance that is 100% employee paid.

Your benefit begins on the 15 consecutive day of disability due to accidental injury or sickness. You will receive
a weekly benefit of 60% of your basic weekly earnings up to $1,000 maximum.

Group Accident – Reliance Standard

Open Sky Wilderness provides you Accident insurance that is 100% employer paid for field guides who are not yet
eligible for the group medical plan or have declined the group medical plan. It is 100% voluntary/employee paid
for all other employees (including field guide) that have elected Open Sky’s group medical coverage. Accident
insurance will pay you a cash benefit when you have a minor accident. The amount you receive is dependent on
the type and severity of injury, and the type of treatment required. See additional plan summary for details.

Hospital Indemnity – Reliance Standard

Open Sky Wilderness offers you Hospital Indemnity insurance that is 100% employer paid for field guides who
have declined Open Sky’s group medical plan, and 100% voluntary/employee paid for all other employees
(including field guides that have enrolled in the medical plan). Hospital indemnity insurance will pay you a cash
benefit to help cover some of the medical costs not covered by primary insurance plan. The amount of payment
is dependent on the condition for which you are hospitalized and how long your confinement lasts. See additional

Group Critical Illness – Reliance Standard

Open Sky Wilderness offers you voluntary Critical Illness insurance that is 100% employee paid. Critical Illness
insurance pays you a cash benefit to use towards medical bills, ongoing treatment, or personal expenses. The
amount of payment is dependent on the condition for which you are diagnosed, and the treatment involved. See

Employee Assistance Program - Triad

Open Sky Wilderness provides you Employee Assistance Program benefits that is 100% employer paid.
When you face a problem or crisis, the Employee Assistance Program offers help and support with a wide range
of issues that many of us face. Up to three face-to-face sessions per year with a licensed mental health provider
