            Voluntary Term Life Insurance

            Team members can select policies on themselves in increments of $10,000 up to a maximum of $500,000. During
            the new hire enrollment window, team members may elect up to the guarantee issue amount of $350,000 for
            themselves or $50,000 for a spouse, with no medical questions. Policies are also available for spouses in increments
            of $5,000 up to a maximum amount of the team member’s approved policy level.


            Policies for dependent children are available in a flat amount of $10,000. Child life insurance does not require a
            medical questionnaire; however, the team member must have coverage on themselves to elect dependent coverage.


            Voluntary term life insurance rates for the team member and spouse are based on the team member’s age and
            increase in years the team member’s age ends in a five (5) or a zero (0).


            Increases or newly elected voluntary life insurance (other than at the time of hire) require the team member and/or
            spouse to complete the Personal Health Application, which can be obtained from Human Resources.
