Eligibility

The amount of support you require determines which of four levels of Home Care Packages you will receive. Each level has a different amount
of funding (subsidy) available. The Australian government pays the provider you choose to deliver your care and services.
My Aged Care
My Aged Care is a contact centre that will assist you to work out what services are available, their costs and how you access them. They will create a personalised client record which will hold up-to-date information on your needs, the results of any assessments and any services that you receive. The client record reduces the need for you to retell your story.
If you’re eligible for a Home Care Package assessment, My Aged Care will refer you to the Aged Care Assessment Team.
Aged Care Assessment Team (ACAT)
Home Care Package assessments are done
by the Aged Care Assessment Team (ACAT)
or Aged Care Assessment Service (ACAS) and Regional Assessment Service (RAS) in Victoria. An ACAT, ACAS or RAS professional will visit you in your home to find out about your individual needs. They assess whether you are eligible for a Home Care Package (or other assistance) and what level of help you will need.
During the assessment visit, the ACAT or ACAS representative (usually a nurse, social worker or other health care professional) will ask questions about how well you are managing your day-to-day life.
The assessment is an opportunity for you
to share what is important to you and what support would make a difference in the way you live. It would be helpful to think about this before the visit.
