題識：師竹老人、李太夫人榮壽敬
造。南無觀音大士，一軀伏願憑
斯，勝因奉資，福壽永庇。菩提之
樹，長游般若之津。弘濟沉淪，勤
除煩障，妙窮諸法，早契菩提。乃
至聞名頂禮，獲福消災，一切迷
方，同歸覺路。雲還生恭介。
