To Whom It May Concern:

You may thus proceed with the installation of the chairlift at your expense and subject to the following
conditions:

1. Obtain permit from the Town of Winslow to install the lift.
2. Qualified craftsmen with a minimum liability insurance of $1 million will install lift.
3. Licensed and insured electrician(s) will make any needed electrical modifications to the building.
4. All work done will be in compliance with applicable building and electrical codes, and will
minimize change or damage to the building.
5. You will be responsible for maintenance of the chairlift.
6. If required by the Association’s insurers, you will secure a certificate of liability coverage in the
amount of $1 million for the chairlift.
7. You will agree to keep the lift at ground level when not in use.
8. You will agree to reimburse the Association for any costs incurred to temporarily remove the
risk if it is determined to hinder moving bulky items for other unit owners on the second level.
9. You will agree to remove the chair lift, or reimburse the Association for its removal, within 60
days from the date when __________________ no longer needs the lift or is no longer a
resident of unit _________.
10. Upon removal of the lift, you will agree to restore the stairway to its original condition (before
the lift was installed), including replacing the stairway carpet if necessary.

Once approved, it is agreed by all parties that the agreement shall remain in force until one or more
conditions discussed above causes the agreement to become null and void.
