Dependent Eligibility

General Description of Eligibility

• Spouse: The employee’s lawful spouse as defined by California law or

“Domestic Partner” as mandated by the State of California.

• Children: The dependent, natural children of the employee or his or her

spouse, including legally adopted children and step children, per the
conditions stated in the Evidence of Coverage.

Each child is eligible until the age of 26. Eligibility continues beyond the

limiting age according to the following:

• Disabled Child

PLEASE NOTE: Not all carriers send out a notification informing you about your
dependent’s over age status. Additionally, as the insured, you are responsible
for providing Human Resources with the verification showing a physician’s letter

confirming the dependent is disabled.
