The PERS Gold medical plan offers numerous health and wellness programs to help you stay on top of your healthcare while
saving money.

PERS Gold is the lowest premium plan available to all County employees while still offering a strong network, low office copays,
and an affordable coinsurance. The PERS Gold plan was designed to keep you more engaged in your healthcare by encouraging
you to participate in more health and wellness programs.

Health Credits
The PERS Gold plan offers you and your enrolled spouse a $100 credit towards your deductible for each eligible program you
complete. For example, if your PERS Gold family plan coverage consists of you, your spouse, and your children, your annual
deductible will start at $2,000. If both, you and your spouse complete each of the wellness and health programs listed below,
you will each receive up to $500 in credits which would bring down your family’s annual deductible from $2,000 to $1,000. You
may choose to complete all the credits, or you may pick and choose which credits to complete.

Flu Shot Receive a flu shot at a pharmacy in your plan’s network, at your doctor’s office, or at the annual
employee flu clinics. Contact Anthem Blue Cross at 1-877-737-7776 to have your credit applied.

Nonsmoking If you do not smoke, complete the health assessment on the Sydney Health app.
Certification If you do smoke, complete a quit-smoking program at no cost.

Complete the test at your doctor’s office or at a Quest Diagnostics Patient Services Center. To
schedule your appointment, click HERE and use registration code CALPERS2020. If a center is not
Biometric near you, order an at-home test kit.
Screening The test checks your blood pressure, cholesterol level, glucose, and A1C, as well as your height
and weight for body mass index (BMI). You will receive your screening results both online and in
the mail.

Call ConsumerMedical at 888-361-3944 (Monday through Friday, 5:30am to 8pm PT) if you are
Virtual Second having nonurgent and nonemergency scheduled surgery in 2023. They will see if you need a

second opinion. You can also receive a second opinion from a PERS Gold plan doctor.

Take part in the ConditionCare program if you have asthma, diabetes, chronic obstructive
ConditionCare pulmonary disease (COPD), heart failure, or coronary artery or vascular disease.
Certification If you receive a letter or call from the ConditionCare team that you qualify to participate, call 866-
962-0957 and join the program to earn your credit.

Please note: Employees and their spouses automatically receive a credit for the Virtual Second Opinion and ConditionCare
program. If the employee or their spouse schedule a nonemergency or nonurgent surgery without receiving a free virtual second
opinion, the credit is forfeited for that individual. If the employee or their spouse has a condition that is part of the
ConditionCare program list and does not enroll in the program, the credit is forfeited for that individual.

Future Moms Program
The Future Moms Program offers the resources you need for a healthy pregnancy, delivery, and baby. After you have a
maternity visit with your doctor, you will receive an invitation to join Future Moms by email, text, or phone call or you can visit

myadvocatehelps.com to enroll. If you enroll in Future Moms before your 36 week of pregnancy, your 20% inpatient delivery
coinsurance will be waived.
