Health Insurance Cards
If you have not received your health insurance card yet you can expect to receive it in the mail by the end
of December. If you do not receive your card by the end of December, please reach out to the insurance
company directly to inquire about its status. Some plans automatically assign a Primary Care Provider (PCP)
at the beginning of the year. Should you wish to change your assigned PCP, you will need to reach out to
your insurance company directly at the phone number below. Typically, PCP change requests are updated
the first of the month after you make the request.

Dental and Vision Insurance
You will not receive new dental and vision insurance cards, however, you can download a digital copy HERE.
Our dental insurance carrier is Pacific Health Alliance, while our vision insurance is provided by Vision Service
Provider (VSP).
Flexible Spending Account (FSA) and Dependent Care Assistance Program (DCAP)
If this is your first time enrolling in FSA and/or DCAP, your FBA MasterCard debit card will be mailed in
December. If you already have an FSA/DCAP debit card, your new 2024 contributions will be loaded to the
same card, and it will remain working until it expires. If your card has expired, or you have lost it, you can
contact Flexible Benefit Administrators (FBA) at 800-437-3539 to request a new card for yourself, your spouse,
or any other dependents 18 years of age or older.
