Save $1,000 on an Individual Deductible or $2,000 on a Family Deductible!
If you complete BOTH the Biometric Screening (1) and
Health Risk Assessment (2) in order and by the designated due date, the Individual In-Network Deductible is $500 and for two or more the In-Network Deductible is $1,000 per family. If you do not complete these activities, in order and by the noted deadlines, your In-Network annual medical plan deductible will not be reduced and will be $1,500 Individual/$3,000 two or more.
