The MRI reveals a herniated disc with nerve impingement and a referral is made
to a physiatrist to check for nerve damage. To rule out nerve damage, the Stanger
Injury Treatment Centers physiatrist (specialist in physical medicine and
rehabilitation) feels it is necessary to perform nerve conduction and
electromyelogram (EMG) studies.
A positive EMG is obtained, suggesting nerve damage and a decision is made at
this point to make a referral to a neurosurgeon for a consultation to assess the
possible need for more invasive pain management or neurosurgical procedures.
At this juncture, the neurosurgeon may decide to administer a trial of epidural
injections or some other type of invasive pain management procedures or may
decide that the only option is spinal surgery.
The above somewhat simplified hypothetical case study illustrates how Stanger
