Regular eye exams do more than Just help you see well.
• Coverage provided by Guardian: www.guardiananytime.com
– Log on to print a vision card, view benefits, coverage, claims, update information
• Coverage for: Employee, Spouse and Child(ren)
• 2 Networks: VSP Choice or Davis Vision
• In-Network Copays: $10 exam / $25 materials
• In-Network Benefits:
– Frames Benefit: $130 retail max + 20% off balance
– Contact Lens Benefit: $130 max for elective contact lenses
• Benefit Schedule:
– Comprehensive Eye Exam: once every calendar year
– Eyeglass Lenses: once every calendar year (in lieu of contact lenses)
– Contact Lenses: once every calendar year (in lieu of eyeglass lenses)
– Eyeglass Frames: once every other calendar year
