Dental Coverage
You do not need to change dentist, you cankeepyoucurrentdentist, asinnetwork and out of network benefits are the same.
• Coverage for: Employee, Spouse and Child(ren)
• 2 Plan Options: Base Plan or Enhanced Plan
• Annual Deductible: $50 Individual / $150 Family
• Benefit Schedule:
– Preventive / Diagnostic Services: covered at 100% under both plans
– Basic Services (after deductible): 50% - Base / 80% - Enhanced
– Major Services (after deductible, 12 mo waiting period): Network Discounts - Base /
50% - Enhanced
• Annual Maximums: $750 - Base or $1,000 - Enhanced (per person per year)
• Plan pays same for in-network and out-of-network providers
– out-of-network may be subject to balance billing • In network and out-of-network benefits are the same
