Dental plan will be moved to Guardian PPO Plans. Dental rates will reduce as follows:
• Coverage for: Employee, Spouse and Child(ren)
• Two Plan Options: Base Plan or Enhanced Plan
• Annual Deductible: $50 Individual / $150 Family (base) $25 individual / $75 family (enhanced)
• Benefit Schedule:
– Preventive / Diagnostic Services: covered at 100% under
both plans
– Basic Services (after deductible): 50% - Base / 80% -
Enhanced
– Major Services (after deductible, 12 mo waiting period):
Network Discounts - Base / 50% - Enhanced
• Annual Maximums: $750 (per person per year)
• Orthodontia (24 month waiting period for enhanced benefit
plan) 50% after deductible-$1000 lifetime maximum per person
• Plan pays same for in-network and out-of-network providers
– out-of-network may be subject to balance billing • In network and out-of-network benefits are the same
