Per 2004 action of the CCCI Board of Directors, the CCCI’s OFA database has been expanded to include
all CCCI recognized health certifications. These certifications include: OFA (Hip, Elbow, Patella, Thyroid,
Cardiac, DNA), PennHip, CERF and CHIC.
When you receive notification of your Chow’s health certification, please send the following information to:

OWNER CHOICE FOR DATA DISCLOSURE
HIP AND/OR ELBOW CERTIFICATION: The Orthopedic Foundation for Animals has always had a semi-open database, only disclosing data for
24 months of age or older normal results. The OFA will now allow owner choice regarding disclosure of data from individual animals.
At the time of submission, the owner must choose if abnormal results are to be released. If no choice is made,
PRELIMINARY HIP AND/OR only normal results will be disclosed for individuals.
ELBOW EVALUATION: Under 24 Months This change will not affect information currently in the database. If an owner would like to disclose abnormal
results on an animal evaluated before this policy went into effect, they may do so by completing and submit-
HIP/ELBOW EVALUATION: ting a form with the owner’s signature. The form is available at www.offa.org/openform.html.
