          Services currently offered by OFA:
          HIP AND/OR ELBOW
          CERTIFICATION:
          24 months of age or older        Per 2004 action of the CCCI Board of Directors, the CCCI’s OFA database has been expanded to include
                                           all CCCI recognized health certifications.  These certifications include:  OFA (Hip, Elbow, Patella, Thyroid,
          PRELIMINARY HIP AND/OR           Cardiac, DNA), PennHip, CERF and CHIC.
          ELBOW EVALUATION:
          Under 24 Months                  When you receive notification of your Chow’s health certification, please send the following information to:
