      1. 46 y.o with multiple leg ulcer, DM, poor compliance and hx of stroke. PCP ordered HH for PT and wound care but d/t  long hx of poor
      compliance, HH agencies refused to admit him. He is now receiving outpatient PT. Pt upset about not receiving HH but I explained to him
      that HH agencies have to comply with Medicare/Medical protocols. He finally agreed to receive wound care at a wound clinic. PCP was
      updated and she agreed to place a wound care referral. I updated the patient and he was appreciative. He has an IHSS C/G who comes
      in 5 days a week.


      2. Three year old male was seen in the ER yesterday for lip laceration after falling face down to his toy truck. Spoke with mom today. She
      does not work. She has food stamps. She just lost her husband last year. They have three children together: 1 y.o, 3y.o and 9 y.o. She
      has no help since her husband's passing. She does have her mom who helps at times. We discussed Child Development Resources and
      WIC. She stated that she does have daycare when she has errands to do and is connected with CDR.  She will call them and apply for
      benefits. She was appreciative of the follow up call.


      3. 40 y.o. needing to see a psychiatrist for insomnia and chronic pain. She was frustrated that Beacon ( GC- mental health services
      providers) have not been successful in connecting her to a psychiatrist for many months now. I provided her two psychiatrists who I
      frequently refer my Gold Coast patients to. She asked how come no one had given her this info in the past. I also provided her Ventura
      COunty BH as a last resort. I can feel the sense of relief in her voice  on the phone. Pt was very appreciative.
