change or rehire to elect medical coverage.
Regular full-time and part-time employees working 30 hours or more per week are eligible to participate in the
Medical Plan with Agency contribution. Regular part-time employees working 20–29 hours per week are
eligible to participate in the Medical Plan by paying 100% of the premium rate.
Spousal Medical Benefits Policy
Employers are constantly struggling with ways to control health care costs while continuing to offer a
competitive and affordable benefit package for employees. Northern Human Services is no exception to this.
Health care cost continue to increase at alarming rates and employers must use innovative methods to control
the increases.
Northern Human Services will allow medical coverage for spouses whose employer does not offer large group
medical insurance (over 50 employees).
If your spouse is not currently eligible for their employer sponsored plan, we will need a letter from their
employer indicating ineligibility for benefits in order to continue/enroll their coverage on the Northern plan.
All newly covered spouses will need to fill out a Spousal Affidavit form. Please direct questions to your local
Office Manager or Human Resources.

$25. Outpatient labs and x-rays are covered in full. Please note that Emergency Room visits go to your

$10,000 per family. There is no carryover of your deductible from the last plan year. There is a Health
