The OptiMed GAP plan provides a dual bucket benefit. These benefits will help cover some of the
out-of-pocket expenses from your major medical plan such as deductibles, co-insurance, and some
copays. Many employees may realize a $0 out-of-pocket cost when including a GAP plan (see policy
for more details and exclusions).

Your Benefits Instructions for Use
$5,000 Inpatient (Per Person) Tell your Provider that you have OptiMed
GAP! Present your OptiMed GAP card with
$2,500 Outpatient (x2 For Family)
your USHL ID card.
Covers:
• Provider bills USHL first.
• Any Deductible and Coinsurance Expense
• Outpatient Surgical • USHL generates an “Explanation of Benefits”
(EOB) and sends to the Provider to confirm
• Diagnostic Testing Facility
payment eligibility
• Office Visits that go towards deductible
• Provider sends EOB to OptiMed.
• Urgent Care & Emergency Room Copays
• OptiMed pays Provider directly.
