patient notified of the results before they are scanned into
the record, or
• If the tests or reports are scanned prior to the physicians
review or uploaded directly into the electronic record, they
should be:
» Flagged as received and tasked to the physician,
thereby necessitating review electronically along with
notification to the patient;
» Reassigned to another provider to review when the
ordering provider is not in the office (i.e. sick, vacation,
etc.); and
» Periodically reviewed by the administrator/manager
to verify that tasks in provider inboxes are addressed
timely.
Paper Records
• Tests or reports should be reviewed, dated, and initialed
by the provider and his/her patient notified of results
(including any follow-up instructions) before they are filed
in the record.
• If the provider who ordered the tests is not in the office
when the results are received, a surrogate reviewer should
be assigned to fulfill this responsibility.
Electronic Tracking System
If you are using an electronic system to track the tests that you
have ordered or referrals that you have scheduled, you need to
ensure that the system can identify which test results have not
yet been received in your office, without having to look at each
