If you are the referring provider:
• Document the reason for the consultation in the patient’s
medical record.
• Explain to the patient the need for, and the advantages of, the
consultation, and document the discussion in the patient’s
medical record.
• Speak directly with the consulting physician whenever possible.
• Provide the consulting physician with complete, accurate, and
timely patient information as well as the reason for the referral
and request written findings.
• Implement a tracking system to verify receipt of these findings.
If never received, contact the consulting physician to verify that
the patient was seen as recommended.
• Train all staff not to file (or scan into the EMR) any reports,
including consultation reports or letters, without the physician’s
review.
• If the patient canceled or was a no-show, contact the patient
to reiterate the reason for the referral and document this
discussion in the medical record.
• Make sure there is a clear understanding as to who is
responsible for the patient’s care.
