the use of any medications they prescribe and set forth
in the PDR, package inserts, product advisories, or other
authoritative resources.
14. Train staff who administer medications to adhere to the
“Five Rights”: right patient, right drug, right dose, right
route, and right time. Staff should read all physician
orders for medication administration back to them prior to
administering any medication.
15. Closely monitor drugs with known toxic effects – use a
“recall” or “reminder” system to alert the practitioner when
labs are due.
16. Periodically re-evaluate a patient’s condition before
renewing medications.
17. Exercise great caution when prescribing controlled
substances, especially narcotics.
18. Regarding sample medication:
a. Monitor drug samples monthly.
b. Keep sample medications in a secured area that is not
easily accessible to patients.
c. Drug storage areas should be well organized with
look-alike or sound-alike drugs stored away from one
another (refrain from alphabetizing the drug storage
areas).
d. All sample medications should be dispensed to patients
by a physician or advanced practice provider.
e. Sample medications should be documented in the
medical record.
f. Staff should be advised that sample medications are
