are triaged. They can also include protocols and methods
applied in the facility where the procedure is performed, such
as the process used in labor and delivery to ensure that there
is an accurate count of surgical instruments or sponges at the
completion of the delivery.
Here is a breakdown of the types of systems issues that were
found in our analysis of closed OB/GYN claims. As will be
discussed later under medical judgment issues, “missed
diagnosis” is a top claim received at SVMIC, and a variety of
systems failures can contribute to these claims.
