6. Click the Form Fields link.
7. Enter the PI Full Name for the lab requesting laundering service.
8. Enter the PI Contact Phone.
9. Enter the Pick-up Address/Building.
10. Enter the Pick-up Room/Location/Aisle. Please be as detailed as possible and be sure to include bench
numbers when applicable.
11. Enter the Quantity of lab coats that you need laundered.
12. Enter the Description. Descriptions may include additional information on how to locate your coats, i.e. laundry bag
will located just outside room #... or "laundry bag will located at the end of bench B", or the numbers from barcode attached to
your coats after initial pickup.
13. Click the Next button.
