Request a Refund – On Account
3. Select the Form Type On Account. The On Account form type is only used by the Controller’s Office.
4. Enter the Award ID (10-digit number) for the OA Item. The remaining award information
will auto-populate. This step is optional.
5. Enter the Customer ID of the entity with the OA item.
6. Click the Look-up to select the Item ID to be refunded.
7. Enter the Total Cash Received.
8. Leave Blank.
9. Leave Blank.
10. Enter the Refund Amount.
11. Remittance Information will auto-populate if Sponsor is linked to Supplier. If blank, AR team
will act.
12. Check the box only if the refund should be sent to an entity and/or address other than the
one displayed in the Remittance Information section. If checked, fields are available to enter the new
remittance information.
13. Enter a Justification. The justification is the reason for the refund.
14. Click the Save button.
You must save the document before adding attachments.
15. Click the Attachments icon.
The File Attachment box opens.
16. Click the Browse button to select any supporting documentation.
17. Click the Upload button to attach the document(s).
