INCOME PROTECTION BENEFITS

        Luminex Home Décor and Fragrance offers a variety of plans to provide replacement income for you or your beneficiaries in the event of disability,
        accident, or death. Coverage is offered through Lincoln Financial (NEW for 2020).

        BASIC LIFE AND AD&D - OFFERED THROUGH LINCOLN FINANCIAL
        Luminex Home Décor and Fragrance provides you with Basic Life Insurance in the amount of of $20,000 and Accidental Death and Dismemberment
        (AD&D) coverage in the amount of $20,000.

        SHORT-TERM DISABILITY - OFFERED THROUGH LINCOLN FINANCIAL
        Luminex offers company provided STD to all full-time employees.This benefit pays a $250 weekly benefit for a maximum of 26 weeks. Benefit
        begins on the 1st day you are unable to work due to an accident and on the 8th day you are unable to work due to an illness.

        OPTIONAL LIFE
         • Employee: You may purchase coverage between 1x to 5x Annual Earnings to $500,000. If you choose a life benefit amount greater than
           $200,000 you will need to have a personal health statement approved by Lincoln Financial.
         • Accelerated Life Benefits: 75% to $250,000.
         • Conversion/Portability option is available upon separation from the company.
         • The guaranteed issue amount is: $200,000.
        OPTIONAL DEPENDENT LIFE
         • Spouse benefit amount: Increments of $10,000 up to $100,000.
         • Child benefit amount: Increments of $2,000 up to $10,000.
         • Dependent elections cannot exceed 50% of employee benefit amount.
         • Participation is voluntary.
        To enroll a dependent in an optional life policy, you must be enrolled in the Employee Optional Life plan.
