INCOME PROTECTION BENEFITS

Luminex Home Décor and Fragrance offers a variety of plans to provide replacement income for you or your beneficiaries in the event of disability,
accident, or death. Coverage is offered through Lincoln Financial.

BASIC LIFE AND AD&D
Luminex Home Décor and Fragrance provides you with Basic Life Insurance through Lincoln Financial in the amount of 1x your salary to a maximum
of $500,000 and Accidental Death and Dismemberment (AD&D) coverage in the amount of 1x your salary to a maximum of $500,000. Your Basic
Life and AD&D coverage is reduced by 35% at age 65 and by 55% at age 70.
SUPPLEMENTAL LIFE AND AD&D
You have the option to elect additional Supplemental Life and AD&D coverage through Lincoln Financial:

• Employee: 1x, 2x, 3x, 4x, or 5x salary up to $500,000.
• Guaranteed Issue: 3x Salary or $200,000.
• Spouse: Increments of $10,000 up to $100,000.
• Guaranteed Issue: $20,000.
• Children: Increments of $2,000 up to $10,000.
**Guaranteed Issue amounts are available only when initially eligible (during the first 31 days of employment).
If Supplemental Life coverage is elected during the annual Open Enrollment period, the application will be subject to Evidence of Insurability and
approved by Liberty Mutual. Benefits will reduce beginning at age 65. Conversion and Portability rights apply upon termination of coverage.
SHORT-TERM DISABILITY

If you aren’t able to work after 7 consecutive days of disability due to an eligible injury or illness, this benefit pays 100% of your base pay for up to
4 weeks; 80% of your base pay for weeks 5-8 and 60% of your base pay for weeks 9-26.
LONG-TERM DISABILITY

This 100% employee paid benefit pays a portion of your income if you continue to be disabled and your Short-Term Disability benefits end. To
qualify, you must be disabled for 180 days. LTD benefits provide you with 60% of your annual base pay up to a $6,000 monthly maximum.
