Know Your Income Protection Benefits
Luminex Home Décor and Fragrance offers a variety of plans to provide replacement income for you or your beneficiaries in the event of
disability, accident, or death.

Basic Life and AD&D - Offered through AnthemLife
Luminex Home Décor and Fragrance provides you with basic life insurance in the amount of $20,000 and accidental death and
dismemberment (AD&D) coverage in the amount of $20,000.
Short-Term Disability - Offered through AnthemLife
Luminex is offering Short Term Disability to eligible employees at no cost. This benefit pays a $250 weekly benefit for a maximum of 26
weeks. Benefit begins on the 1st day you are unable to work due to an accident and on the 8th day you are unable to work due to an illness.
Optional Life

*If you are currently enrolled in an optional life plan and would like to increase your benefit value, please see your local HR contact for enrollment
instructions.
• You may purchase coverage in an amount from 1x, 2x, 3x or 4x your annual salary to a maximum of $500,000 or 4x salary in increments of
$10,000. If you choose a life benefit amount greater than $200,000 you will need to have a personal health statement approved by
Anthem Life.
• Accelerated Life Benefits: 75% to $250,000
• Conversion/Portability option is available upon separation from the company
• The guaranteed issue amount is: $200,000
Optional Dependent Life

*You are unable to enroll dependents in an optional life policy unless you are enrolled as an employee.
• Spouse benefit amount: Choice of $5,000 or $15,000
• Child benefit amount: $2,000
• Dependent elections cannot exceed 50% of employee benefit amount
• Voluntary participation
