Group Benefit Offerings

BCBSM Simply Blue PPO Platinum

$250 Single/$500 per family deductible followed by an 80/20 coinsurance to
an embedded coinsurance maximum of $1000/$2000 then to an overall
maximum of $6600/$13,200 per calendar year, $20 PCP Office Visit Copay,
including Online Visits, $40 Specialist Copay, $60 Urgent Care Copay, $150
Emergency Room Copay, $10 Generic/$40 Preferred Brand Name/$80 Non-
Preferred Brand Name/15% Preferred Specialty Drugs/25% Non-Preferred
Specialty for Prescription Drugs

BCBSM - Dental

The Dental plan covers 100% of Preventative Services, 80% of Basic Services,
50% of Major Services, and 50% of Orthodontia Services. $25/$75 per family
deductible. Deductible does not apply to BASIC, MAJOR or ORTHO services.
Each person on the plan has $1,000 of annual benefit running on a calendar
year basis, and a $1000 lifetime maximum for Ortho.

Eye Med - Vision

The vision plan has a $180 allowance for Frames or Lenses. You have a $10
Copay for Exam and Materials. You are eligible for these benefits once every
12 months.

UNUM - Life Insurance

PAAM provides all full-time employees with 2 x annual salary to a maximum
of $100,000 of Term Life Insurance and Accidental Death and
Dismemberment. $100,000 is Guaranteed Issue (No medical questions
asked)

UNUM - Long-Term Disability

Should you not be able to return to work after 90 days, LTD will cover 60% of
monthly earnings to a maximum of $6000/month, EAP, Pre-Ex: 3/12
