Dear Employee,
Affiliate Mortgage Services is excited to continue to offer benefits to our full-time employees.
Rather than offering a single health plan, we are offering two plans; so you and your family can
choose the plan that best fits your needs.
Affiliate Mortgage Service is paying 75% of the premium for the Blue Care Network HMO Gold
$1000 plan (Base Plan). Employees are responsible for the difference in premium if choosing
the BCN HMO Platinum 20% plan.
Open enrollment begins on Friday, August 7th through Sunday, August 16 th
