Prevention Forward Healthy Lifestyles Program
Northridge Hospital’s Center for Healthier Communities and the Chronic Disease Transitional Care Team facilitates the California Department of Public Health Prevention Forward Program. Free health and education workshops are provided
to community members who have pre-diabetes, diabetes, cardiovascular disease, high blood pressure or stroke. Medication management is also provided by pharmacists. In FY22, 1,047 individuals were educated at community outreach events and workshops. They learned how to self-manage their conditions,
improve outcomes, and prevent setbacks in recovery. Participants in the program also received blood pressure monitors, glucometers, a stress ball, and resistance bands to start their heart health journey.
Helping Victims of Elder Abuse
The Center for Healthier Communities is in its third year of implementing the Local Elder Abuse Prevention Program (LEAP) grant, thanks to a three-year grant totaling $375,000 from the U.S. Department of Justice, Office of Victims of Crime. The grant helps vulnerable older adults who are victims of elder abuse and financial exploitation by linking them with services appropriate to their specific needs. In addition, the grant helps provide awareness and educational outreach to community residents and professionals who are mandated reporters.
In FY22, our LEAP Enhanced Multidisciplinary Team (EMDT) met virtually once each month to conduct new and existing elder abuse case reviews for clients referred to us. To date we have reviewed 16 cases. Our EMDT also provided 71 virtual elder abuse prevention educational workshops to 1,233 participants, conducted in English and Spanish by our sub-grantee, Alzheimer’s Association California Southland Chapter. An additional six workshops were conducted by Bet Tzedek Legal Services for the LA Family Housing for 759 professionals who work with older adults, in addition to a workshop at Northridge Hospital that reached 24 staff members.
We also participated in one community awareness event at the ONEgeneration Senior Symposium, providing educational materials to 335 individuals. We closed out this year of the grant with a social media marketing campaign during the week of June 15, 2022, to coincide with National World Elder Abuse Day. Vital members of our EMDT also include the Alzheimer’s Association California Southland Chapter, Adult Protective Services, WISE and Healthy Aging Long-Term Care Ombudsman.
New Endoscopy Procedure Room
With lung cancer the most common cancer worldwide, the Northridge Hospital Foundation is pleased to help expand
the recently established Interventional Pulmonology service line with a $355,155 gift. Northridge Hospital
has utilized these funds for the
expansion of a completely remodeled
modern endoscopy procedure suite.
The new suite is equipped with
negative air pressure, additional space
for the surgical team, and specialized equipment to perform a minimally -invasive technique called electromagnetic navigational bronchoscopy, which can go farther into the lung nodules to improve treatment for
lung cancer patients. What’s more, the hospital can also utilize this room as a second Gl suite, when not in use, to further support our gastroenterology patients.
IMPROVING MEDICAL CARE
Lifesaving Automated External Defibrillators
Automated external defibrillators (AEDs)
are devices that send an electric pulse or shock to the heart to restore a normal heart- beat. They are used to prevent or correct an arrhythmia, an uneven heartbeat that is too slow or too fast. If the heart suddenly stops, defibrillators can also help it beat again. Thanks to your generosity, the Northridge Hospital Foundation was able to purchase 75 AEDs to ensure they are readily available on each and every floor. We were also able to purchase new AEDs for our cardiac and other areas.
