GROUP BENEFIT OFFERINGS
BCBSM Simply Blue PPO H.S.A. – Mapped Medical
$2000 Single Deductible, $4000 Family deductible followed by
100% coinsurance to an overall maximum out of pocket of
$3000 Single, $6000 per family per calendar year for all medical
services, $20 Generic/$60 Preferred Brand Name/50% Non
Preferred Brand Name/20% Preferred Specialty/25% Non
Preferred Specialty Prescription Drugs after deductible has been
satisfied. (Previous: $1500/$3000 deductible and $2500/$5000 Out of
pocket).
Guardian- Life Insurance
Kheder Davis provides all full-time employees 2 x annual salary
of Term Life Insurance and Accidental Death and
Dismemberment up to $150,000.
Guardian - Long-Term Disability
Should you not be able to return to work after 90 days, LTD will
cover 60% of monthly earnings to a maximum of $3000/month.
Guardian rates only increase based on the age band
A Pediatric dental plan is set up for dependents up to the age of 19 with
Delta Dental. Currently no one is enrolled.
