Group Benefit Offerings

Delta Dental - Dental

This plan covers 100% of Preventative Services, 80% Basic Services,
50% Major Services, and 50% of Orthodontia Services. Each person
on the plan has $1000 of annual benefit running on a calendar year
basis, and a $1000 lifetime maximum for Ortho. $50 per person/$150
deductible per family. Deductible does not apply to Preventive or
Orthodontic Services.

No rate increase

Eye Med - Vision

The vision plan has a $180 allowance for Frames and Lenses. You
have a $10 Copay for an exam and a $25 Copay for materials. You are
eligible for these benefits once every 12 months.

Rates are guaranteed 9/1/19 – 9/1/23

Guardian- Life Insurance

ECIC provides all full-time employees with 2 x salary to a maximum of
$200,000 (Class 2) – Guaranteed issue $200,000 and 2 x salary to a
maximum of $300,000 (Class 1) – Guaranteed issue $215,000 of Term Life
and Accidental Death and Dismemberment.

Guardian – Voluntary Life

Employee: $100,000 – GI $100,000

Spouse: $25,000 – GI $25,000

Child: $10,000 – GI $10,000

Guardian – Short Term Disability

8 day accident, 8 day sickness to a duration up to 12 weeks, 60% of
weekly earnings to a maximum of $1000/week.
Guardian - Long-Term Disability

Should you not be able to return to work after 90 days, LTD will cover 60%
of monthly earnings to a maximum of $5000/month with a Two -Year Own
Occupation, payable to SSNRA
