Dear Employee,

Habitat for Humanity of Michigan is again excited to offer benefits to our full-time employees.
Rather than offering a single health plan, we are offering two health plans; so you and your
family can choose the plan that best fits your needs.

The PPO plan with BCBSM, is the full national network of doctors and hospital and does not
require a referral to seek in-network services. The HMO plan with BCN is the state-wide
network which includes every hospital in the state and majority of physicians. If you enroll in
BCN, you must coordinate care with your Primary Care Physician and request a referral to seek
specialist services. Also, if you are traveling, you have nationwide emergent and urgent services
covered as in-network on any plan you choose.

Habitat for Humanity of Michigan is paying 75% of the premium for the Blue Care Network
HMO Gold $1000 plan (Base Plan). Employees are responsible for the difference in premium if
choosing the BCBSM SB PPO Gold $500 plan.
