The family would like to acknowledge their profound

gratitude for the wonderful care given to our mother

by the Nurses and staff at State Street Assisted

Living, as well as for the compassionate care that has

been provided by Seasons Hospice.

In lieu of flowers, the family requests donations to be

made to the Alzheimer’s Association.
