WHEN Should You Call
Rapid Response?
Call Rapid Response if you notice:
• Achangeinheartrateordrop in blood pressure
• Achangeinrespiratory(breathing) rate or oxygen levels
• Achangeinurineoutput(more or less urine)
• Achangeinmentalstatusor level of consciousness
• Anychangeinthepatient’s condition or if you have serious concerns
HOW Do You Call Rapid Response?
• Dial 5555 on bedside phone.
• Tell the operator: your name,
patient’s name, room number
and your concern
• The Rapid Response Team
will be sent to the room.

Hospital Rapid Response Team
Anyone that has contact with a patient, including family members or visitors have access to a special service called the Rapid Response Team. If you notice any change in the patient that concerns you, call
this service to request an immediate evaluation
of the patient.
Our critical-care team will check on you or
your loved one and provide help before there is a life-threatening emergency.

What is HCAHPS?
HCAHPS is the Hospital Consumer Assessment of Healthcare Providers & Systems. It measures patients’ thoughts about their hospital experience by asking questions based on communication, responsiveness, cleanliness, quietness, pain management, communication about medicines, discharge information and overall hospital rating.
