This Guardian Approval & Authorization Form (“Form”) is entered into by the undersigned
parent or legal guardian (“Primary Guardian”) of the minor child (“Talent”) named below, in
BKT MEDIA
CONFIDENTIAL
favor of Baby Kid Teen Media Group (“Media Group”).

1. Authority to Represent
I hereby affirm that I am the legal parent or guardian of the minor listed below and grant Baby
Kid Teen Media Group permission to communicate with me regarding all matters related to
Talent’s participation, including:

 Scheduling, booking, and travel coordination
 Media usage, training, and performance opportunities
 Legal forms, releases, and documentation

I further authorize myself to book travel, lodging, and services on behalf of Talent, and to
speak and make decisions on their behalf in all Media Group-related matters.

2. Wage & Compensation Handling

I understand that any wages, stipends, or approved compensation earned by Talent through
Media Group opportunities will be paid to me on behalf of the minor, unless otherwise
arranged in writing.

3. Additional Guardian (Optional)

If applicable, I designate the following individual as an Additional Guardian authorized to
receive confidential information and act on behalf of Talent when necessary:
