Patient Goals
The patient goal would’ve been entered during the Start of Care visit. The clinician must add any additional
comments to the goal in the lower text box, especially if the original SOC goal is not rehab related.
Diagnosis
Clinicians are required to enter diagnoses in this section that will justify the rehab need. You can copy the
homecare diagnosis from Medical Records, or enter a “treastment diagnosis”, e.g. “muscle weakness”, or
