Process for Discontinuation of Therapy, and Discharge from

Agency

1. Contact MD to obtain D/C verbal orders, and document conversation in a coordination note.

2. Make sure you have a PT/OT/ST 19 (DC of Services Not from Agency) visit plotted in the
patient’s calendar.

3. Complete the PT/OT/ST 19 visit note. Note: If the patient has Medicare insurance, make sure to
have them sign a HHCCN. It can be found under Electronic Forms in Visit Actions. The paper
copy for the patient can be found in their SOC folder. If you plotted a buddy code, the system will
remind you to have a HHCCN signed.

4. If you are the last skilled discipline, you must perform a PT/OT/ST 18 (Discharge From
Agency) visit instead of a 19 visit. This visit will contain the Discharge from Agency OASIS. Note:
If the patient has Medicare insurance, make sure to have them sign a Notice of Medicare Non-
Coverage (NOMNC) at least 48 hours prior to the discharge. It can be found under Electronic
Forms. The paper copy for the patient can be found in their SOC folder. If you plotted a buddy
code, the system will remind you to have a NOMNC signed.
