SOFA Questions 22 Through 24 — Environmental Information

               Requires the listing of certain environmental information about the debtor’s operations. Note that this
               question is notice driven. Reporting is required if the debtor has received or provided notice of certain
               environmental issues or is party to any judicial proceeding concerning environmental laws. Although the
               environmental questions asked on the bankruptcy petitions are slightly different (imminent and identifi-
               able harm to public health or safety), the response to these questions should be reconciled to the petition.

        SOFA Question 25 — Businesses in Which the Debtor is an Owner

               List details regarding businesses in which the debtor was a partner or owner within the six years prior to
               the petition date. Note that the question calls for terminated ownerships.

        SOFA Question 26 — Keepers of Financial Statements

               List all those who have maintained books, performed audits or to whom financials have been issued dur-
               ing the relevant two-year period.

        SOFA Question 27 — Inventories


               List the last two physical inventories by location, and name of persons holding records of such invento-
               ry. If a nonphysical inventory methodology is employed (that is, cycle counts), that methodology should
               be described and the results of such inventory included in response to question 27.

        SOFA Question 28 — Current Partners, Officers, Directors, and Shareholders

               If the debtor is a partnership, the preparer should list each partner and his or her percentage ownership of
               the partnership. If the debtor is a corporation, the preparer should list all current officers, directors, and
               stockholders of the debtor.

        SOFA Question 29 — Former Partners, Officers, Directors, and Shareholders

               If the debtor is a partnership, the preparer should list each partner who withdrew within one year of the
               petition date. If the debtor is a corporation, list all officers or directors whose relationship terminated
               within one year of the petition date.

        SOFA Question 30 — Payments, Withdrawals, and Distributions by Corporation or Partnership

               List all payments and withdrawals from a partnership or corporation and distributions (dividends and so
               on) from a corporation, whether in cash or property, within one year of filing. Generally, the response to
               the question is based on consolidated information to provide one response listing all payments or distri-
               butions to a given insider.


        SOFA Question 31 — Tax Consolidation Group

               List the entity name and tax identification number as listed on the debtor’s federal tax return if part of a
               consolidated filing within six years of filing.
