Accident procedure
Please prepare the following when making the call. Is the vehicle drivable?

Are you ok? Anyone hurt?
Call 911 if needed
Are you out of traffic
Turn hazard flashers on
Put your triangles out
Grab accident report form in your permit book
o Fill the blanks in of the information requested Tractor#, Trailer#, Driver name
Location
o What highway/road, mile marker, closest city/town

o Shoulder of the road or ramp - need exit# If damage was caused please take pictures.

o Minimum of 5 pictures
o Different angles
o Close up picture
o Wide area of damage
o T ake pictures of the other vehicle or property damage

o Do not give form to other driver to fill out.
Get other drivers info - either take pictures of info or write it neatly.
If I have questions regarding handwriting I will be asking you
•
o Drivers name, license# and state, phone#
o Insurance information - can take picture of insurance card
•
Insurance agency, phone#, policy#
o Are there other parties involved
•
Get all of their info as well
o Other vehicle
•
License plate# & state, make/model, color of vehicle
DOT#, MC#, Company name, company contact person & phone# If police arrive get the following
o Case#
o Officer badge#
o Are you cited for accident? - possible post-accident drug/alcohol screen is required
