Covid-19 Attestation

• The purpose of this attestation is to ensure that staff

and volunteers understand their obligations and
responsibilities with respect to maintaining knowledge
of the COVID-19 symptoms as identified by Unity

Health Toronto. By signing off on this attestation, you
are agreeing that under no circumstances will you
report to volunteer if you are experiencing any of the
COVID-19 symptoms.

• In other words, by completing the sign off, you are

confirming that every time you enter Unity Health
property you are asymptomatic.

• Once signed, you will be able to self-screen at home
and then enter the hospital simply by showing your
badge provided you are symptom free.

• You will find a list of symptoms in the Keeping Safe
chapter along with the Covid-19 Call Centre Number to

contact should you have any of the symptoms.

• You will be given a chance to ask questions and asked
to sign a copy on your first shift.
