Fig. 1.20 Combined monitor/CCU in use during laparoscopic liver biopsy.
The lead surgeon is to the left and is looking cranially over the patient to
the monitor.
surgeon may have to move around the table to operate with the domi-
nant hand and to examine the entire body cavity, and look at the monitor
without straining. This may need the tower or monitor to be moved.
Having the image on more than one monitor considerably reduces the
need for repositioning of equipment and hanging the monitors on adjust-
able mounts (on the wall or from the light pendants) is also helpful.
When the procedure is in progress the lights will need to be dimmed and
thus avoiding cable cluttering is important to prevent tripping hazards.
Electrical interference when the electrosurgical unit is in use can be
minimised by using separate electrical outlets for the video cart and the
electrosurgical unit.
For laparoscopy, the insufflator should be positioned at eye level, close
to the video monitor; this allows easy monitoring of the intra-abdominal
pressure. Both the surgeon and anaesthetist should be able to see the
insufflator settings.
