Millennium’s Accountable Care Organization (ACO) is a multidisciplinary team of primary care
physicians, hospitalists, discharge planners, nurse case managers, transitional care manag-
ers and nurse practitioners. Our goal is to empower patients to achieve their maximum health
potential through care coordination, utilization of available community resources, education,
and improved provider access. Millennium Physician Group’s Chronic Care Management (CCM)
services are part of the nationwide program which was created by Centers for Medicare and
Medicaid Services for Medicare beneficiaries diagnosed with two or more chronic conditions
expected to persist at least 12 months or for the remainder of the patient’s life. These condi-
tions typically place a patient at significant risk of death, acute exacerbation, decompensa-
tion or functional decline. These CCM services are designed to provide care management to
patients with specific chronic medical conditions, depression or those with over-burdened
