TEAM MEMBER HANDBOOK ACKNOWLEDGEMENT FORM

               I  hereby  acknowledge  receipt  of  the  Team  Member  Handbook.   I  understand  that  this
               handbook does not constitute a contract of guaranteed or continued employment and that I
               am an at-will team member.  As an at-will team member, I may terminate my employment at
               any time for any reason or for no reason at all.  Likewise, Jack and Jill Children’s Center may
               terminate my employment at any time for any reason or no reason at all.

               The Team Member Handbook, as given to me, is not a written contract of employment, and I
               have  received  no  assurances  or  other  statements  that  indicate  otherwise.  I  further
               acknowledge that Jack and Jill Children’s Center reserves the right to make changes to the
               policies,  procedures  and  benefits  contained  in  the  Team  Member  Handbook  as  it  deems
               appropriate,  and  these  changes  may  be  implemented  even  if  they  have  not  been
               communicated, reprinted, or substituted in this handbook. Only the Chief Executive Officer
               can approve changes to the policies, procedures and benefits contained in the Team Member
               Handbook in writing.  Any other person who attempts to alter or make changes to the policies,
               procedures,  and  benefits  contained  in  the  Team  Member  Handbook  does  not  have  the
               authority to do so.

               I acknowledge receipt of the Team Member Handbook, and I agree to read and abide by the
               policies set forth in the Team Member Handbook.
