Welcome to the E. Cohen 2021 Allstate Benefits Electronic Flipbook. This e-brochure  will
                provide you with an overview of the Allstate Supplemental Employee Benefits made available
                to you through your employer. These benefits are completely voluntary, and are partially
                employee funded through payroll deduction. E. Cohen provides $240/year ($9.23/pay) to
                put towards any Allstate Benefit so there is at least one policy everyone can get for
                free (the supplemental Accident Plan 1). By enrolling through your company, you will enjoy




                greatly reduced group rates, and many   benefit premiums qualify for pre-tax payroll deduction




                status as part of your company’s Section 125 Plan.
                Benefits Offered Include:
                     Short-Term Disability Insurance (includes Maternity Benefits)
                     Term to Age 100 Life Insurance
                     Supplemental Insurance Plans (to help with deductibles, co-pays, out-of- pocket
                      expenses and lost income):
                          o  Hospital Indemnity Plan
                          o  Accident Plan (Free individual coverage for all employees)
                          o  Cancer Plan
                          o  Critical Illness Plan

               Most benefits are offered on a “Simplified Issue” basis (a few medical background questions)
               without the need for a physical exam. Many benefits are initially offered on a "Contingent
               Guaranteed Issue" basis (only 1 medical question) which makes qualification highly likely.
               Please note that these relaxed underwriting criteria are only available to you during
               your initial eligibility period. If you decline coverage at that time, and later decide that you
               would like to enroll, you will have to qualify by answering more medical background
               questions, the answers to which may determine if you are accepted or declined. So, please
               make sure you look at these benefits and  take advantage of the relaxed underwriting period if
               you are interested.
